Please send both forms with your payment to Judy Anderson. Her address is on the payment form and below.

PLEASE NOTE:

• We cannot process your application without both forms.
• Only full payment will secure your position.
• Maximum of 8 students per faculty member.
• The turnaround time from receiving applications and mailing the information packet is generally almost two weeks.
• If applying after July 12, please contact us immediately and we will make every attempt to accommodate your application.
