Application and Payment Process

In order to apply for CAP I and CAP II, you must complete the application form and payment form. 

Please allow 7-10 days to receive a confirmation letter and information packet.  The turnaround time from receiving applications and mailing the information is generally almost two weeks.

Please send both forms with your payment to Judy Anderson – her address is on the payment form and below.  We cannot process your application without both forms.  Thank you.
