Application and Payment Process

To apply to the Piano Academy, submit the following items:
- Completed application form and completed payment form
- Non-refundable $70 application fee
- List of repertoire studied during the past two years. Indicate pieces that have been memorized or performed in recital.
- CD, DVD, VHS videotape, or audio cassette tape of your performance of two pieces of contrasting character and style. If you attended the Piano Academy in 2008 or 2009, an audition tape is not required unless explicitly requested by the Director.
Please send both forms with your payment to Judy Anderson – her address is on the payment form and below. We cannot process your application without both forms. Thank you.

- The $70.00 application fee is non-refundable and
not part of the tuition cost. All applications must include application fee.
- Late applications will be accepted on the basis of available
space.

*There is a $35.00 cancellation fee for refunded tuition.

The Jacobs School of Music reserves the
right to cancel a workshop if enrollment is not met. Participants
will be informed 2 weeks before the start of the workshop and
receive a full refund.
